1. GENERAL RULES OF THE 1+5/1+9/1+13 SCHEME
1.1 Maximum age at entry and / or nominated family members is under 75 years. If dependent is over 75 years, they will be covered as extended member.
1.2 Cover will be provided for a maximum of 5/9/13 nominated family members, (e.g. spouse, children, dependent children, extended family members)
1.3 Application form must be fully and accurately completed by the Policyholder, should any of the information provided not be accurate, membership under this policy may be cancelled and no claims under this plan will be considered for payment.
1.4 Once the principal member’s cover ceases, the policy can be taken over by any nominated family member.
1.5 All dependents are seen as individual members, therefore each member will have a waiting period.
1.6 In case of a member that is expecting a child: make sure that there is space available to add the new child.
1.7 All new born children must be added to the society as new members. They will have a new waiting period starting from the date of policy amendment.
1.8. NB: A member cannot be replaced if he/she passes away.
1.9 If Kamoka Reakgona T/A Corbis is to conduct funeral for the dependants, we use our own coffin and we do not allow the family to bring their own coffin.

2. DOCUMENTS REQUIRED
When a member joins the Burial Society he/she must provide the Burial Society/ Club with the following documentation and information:
3. NATURAL DEATH
A waiting period of 6 (six) month after the receipt of the 7 (seventh) premium will apply calculated from the inception date due to natural death and thus no benefit will be paid until waiting period is completed. Death due to suicide, self-inflicted or deliberate injury will have a 24 months waiting period. If you skipped then the 24 months waiting period will resume from the date you re-instate your policy.
4. MEMBERSHIP
4.1 The main member is allowed to register 14 (fourteen) dependents as member. The 14 (fourteen) dependants will be calculated as follows the member, spouse, members children and parents. If the people you registered are less than 14(fourteen), you can add other family members to make a total of 14(fourteen) members.i.e. parents of the main members spouse and grand children. If a member has more than 14 dependants, a member is allowed to add 4 extra members, but each will be charged individually.
4.2 Therefore the client is obliged to visit the office at least after every 12 month to update his/her policy. The office will also notify the client of any changes by sms or other communication.
4.3 Friendly society/club member benefits are renewable annually. Therefore every society/club must renew their membership certificate every year before the expiry date.
5. CLAIM RATIOS:
If the claim ratio for the Burial Society / Club ,friendly society or per member is more than 70% (SEVENTY PERCENT) or if the Burial society club ,friendly society or per member has too many funerals or claims then the monthly payments will increase from 10% upwards.
Claim Requirements
Fully completed, signed and stamped claim form, Copy of the deceased’s identity document certified, Copy of the death certificate certified, Copy of the main member’s identity documents certified, copy of the BI 1663 forms (4 pages), copy of the Burial Order, Copy of the premium schedule, If the cause of death is unnatural- a copy of the police statement is required.
Kamoka Reakgona T/A Corbis must be notified of a claim within 3 (three) months of a member’s death. A fully completed claim documents must be submitted to our office before 10h00 in order to process and send it to the claims department in time for the claim to be paid within the next 24 hours.
CASH CLAIM FOR 1+5/1+9/1+13 SCHEME – Members who joined from 2001 until 10 September 2019
Our aim is to conduct funeral for you and not to give out cash, therefore if Kamoka reakgona T/A Corbis does not do funeral for you, we will not give you any cash if death occurs within 1 year of joining and 75% of the funeral value if death occurs after 1 year of joining the scheme.
CASH CLAIM FOR FRIENDLY SOCIETY / CLUB / MEMBERS – Members who joined from 11 September 2019
Amount payable if we don’t conduct funeral for the covered individuals) if the member has less than 1 year of membership then we conduct the funeral only, we do not give out cash claim.
If you have more than 1 year we give you 50% of the total funeral value for Member, spouse and Member’s children and EXTENDED FAMILY MEMBERS.
Please note that if the cover amount changes to a higher amount then there is a waiting period for the higher amount.
If the cover is changed to a lower amount then there is no new waiting period if the original 6 months is already completed.
If the member changes to entire new product then the client will be bind by the terms and conditions of the new scheme. For example if the member moves from 1+13 to Friendly society scheme, they will receive benefits of the new scheme they changed to and forfeit the benefits of the old scheme.
6. Repatriation of mortal remains benefit:
6.1 A friendly society/club/member may contribute for the repatriation premium. Premium for the Repatriation is R15.00 per member per month. If a friendly society/club/member contributes the R15.00 per month, then Corbis will not charge the family for extra transport should the removal occur outside our local area.
6.2 If a society / club / member do not pay the repatriation fee then should death occur outside our local radius, the family of the deceased will be obliged to pay for extra transport which is calculated by R6.00 per kilo. Our local radius is less than 100km from our mortuary.
7. CANCELLING MEMBERS AND APPOINTING NEW MEMBERS:
7.1 if the main member would like to replace an existing member, a waiting period of six months will be applicable to the new member.
7.2 If the main member passed away or if the main member swaps with the dependant that is already on the application form, then that dependent who is replacing the main member will not have a new waiting period.
7.3 The cost incurred by the main member to cancel and enrolling a new member will be for the account of the main member, an administration fee of R50.00 (FIFTY RAND) is applicable.
7.4 The cancelling of members and appointment of the new members must be done in person and in writing at the Burial Society Club offices , alternatively by fax 086 538 2736 (on fax please fax client/member application form & id copy of the member-amended member and include the relationship between the new appointed member and the main member).
8. RESIGNING AND CANCELLATION OF MEMBERSHIP:
8.1 The main member must pay the whole amount of the joining fee within 6 (SIX) month of joining, if the main member pays half of the joining fee when joining, then from the second month the main member is required to pay the remaining amount of the joining fee plus his/her monthly premiums in order to be up to date with payments.
8.2 Payments are made on a month to month basis on from the 1st till the 29th of every month, if a client skips one month payment then they must pay double the next month and if they skipped two months they must pay the whole premiums before the last date of the third month to avoid lapsing of the society. Should death occur within the two months that you skipped, you will be required to pay the premiums for three months plus premiums for three months in advance. When you skipped you are required to pay all outstanding premiums at the office before making a claim or funeral arrangements. No skip for three (3) months is allowed, should you skip for three months or more, a six months waiting period will apply after the receipt of the seventh premium.
8.3 On reporting the death or catering to the burial society or club it is required that the family of the deceased must attend in person to the Burial Society or Club.
